FT/PT OD needed inside new COSTCO!

OD wanted to join/partner in new practice opening inside the new Syracuse, Utah COSTCO location! 
***Opens in June and can start immediately
***OK to be independent contractor or employee
***Enjoyable work environment aimed at giving autonomy, flexibility and practicing fuller scope Primary Care optometry per comfort level
***FT or PT with flexible schedule availability. Fill in available PRN as well. 
***2 fully equipped lanes with all new equipment including Marco digital phoropters, OPTOMAP, Oculus Visual Field Analyzer and simple to use EHR. 
***Compensation is negotiable and can be flexible. Base guaranteed per diem plus daily production bonuses or percentage based depending on preference. Highly Lucrative possibility! 
***Pretesting, Insurance billing, contact lens trainings all taken care of.
***Great safe area to live and raise a family! Only 30 minutes from Salt Lake City and located in a new, very nice and rapidly growing city/area in northern Utah. Close to lakes, mountains, several ski resorts and major Intl. airport.
